TMC Health Insurance Forms

The name of the school insurance is Anthem Blue Cross Life and Health Insurance Company (PPO). When you visit a Preferred Provider, you generally have fewer out-of-pocket expenses; however you may see any doctor.

To find a doctor or provider call (800)888-2108 or visit Preferred Provider Option (PPO) at anthem.com/ca.

Students that purchase The Master's College insurance will receive the following in their mail box at the beginning of the semester:

1. Insurance I.D. card (Same card for medical care and prescriptions)
2. Summary of insurance benefits (to view the brochure, go to TMC Insurance Information)
3. The names of local doctors in the area
